Good Faith Estimate

Isozo Wellness is a self-pay practice at launch. Under the No Surprises Act, you have the right to receive a Good Faith Estimate explaining how much your health care will cost before you receive services. This page explains our current self-pay rates and your rights.

Your rights under the No Surprises Act

• If you are uninsured or choose not to use insurance, you have the right to receive a Good Faith Estimate of the total expected cost of your care, including related costs like medical tests or prescription drugs, before a scheduled item or service or any time you ask for one.
• If you schedule a service at least 3 business days in advance, you will receive your Good Faith Estimate within 1 business day of scheduling. If you request an estimate without scheduling, you will receive it within 3 business days.
• Your Good Faith Estimate is not a contract and does not require you to get any items or services from us. Your final charges may differ if your care needs change.
• If you are billed $400 or more above your Good Faith Estimate from any provider, you can dispute the bill through the federal patient-provider dispute resolution process.
• Keep a copy or photo of your Good Faith Estimate for your records.

How to get your estimate

You will receive a Good Faith Estimate when you schedule care with us as a self-pay patient. You can also request one at any time through our patient inquiry form or the patient portal.
